Your scholarship has certain requirements that you must meet each year/semester to remain eligible for ongoing payments.

Each scholarship recipient has the opportunity to submit a written statement and provide appropriate evidence explaining any extenuating circumstances that may have impacted upon their ability to meet the ongoing requirements.

Please complete the form below and upload your written statement along with supporting documentation by the date indicated on the email sent you. The Scholarships Office will consider the provided documentation and you will be advised of the outcome of their decision via your student email.

This statement should address the donor/Executive Dean and detail any extenuating circumstances that impacted upon your ability to meet the ongoing requirements of your scholarship.

Please provide appropriate evidence that supports the circumstances you detailed above. Evidence must demonstrate how circumstances have been impacting your capacity to meet the ongoing scholarship requirements during that study period.
