This form is used by current and past students to submit a request for an academic transcript and authorise a credit card payment (MasterCard, Visa and American Express). If you studied with ACUcom do not complete this form, you can contact ACUcom to request an academic transcript.
Completed your course?
A complimentary transcript will be mailed to you once you have been assessed as course complete by your Course Coordinator. You will receive email notification to your ACU student email account when you have been made course complete and your transcript has been sent.
Changed your name?
Before you request a transcript, you will need to complete the CN: Change of Name application form, along with certified documentary evidence to have your name officially changed on your ACU record.
