This form should be completed by ACU students wishing to return to the University following a period of termination/exclusion/suspension. Use this form to either:
Readmission at ACU is subject to approval by the relevant Head of School, and will be based on your demonstrated capacity for improved performance in your ACU studies. You should provide supporting documentation in addition to your written statement on this form. If you are applying for a new course, you must also meet the relevant prerequisites and entry requirements.
Note: Some courses do not offer a Semester 2 intake. Check for course availability before submitting this form.
