Support program for stroke survivors and their carers

Stroke survivors and their carers are invited to participate in a research project providing support through the Optimal Health Program (OHP) - a 9 week program at St. Vincent’s Hospital.
The OHP focuses on:
Enhancement of self-efficacy and independence
Goal setting and skill building
Working with and learning to navigate health care services
Personalised individual sessions with a healthcare professional
Available face to face, over the phone or via Skype.This research program is available to English-speaking adults who have experienced a stroke or care for someone who has. Please pass this information on to anyone you feel may be interested.
