Reporting Incidents and Injuries on riskware

Staff members, students, contractors and visitors regularly identify hazards and manage risks within working and learning spaces across the University. Safety is everyone’s responsibility and any safety issues should be reported as a matter of priority.

Log a report on riskware

Login using your ACU network user ID and password

Staff and students should use ACU’s online riskware portal to log reports of all incidents, injuries and hazards (something that could potentially cause harm) which they have witnessed or been affected by. These incidences may occur on or off campus, but all should be reported promptly – simply login to riskware and fill in an easy-to-use report.

If a staff member is injured as a result of a work activity, they should contact Human Resource’s WHS staff – or ext.4222 – to discuss their workers’ compensation options.

All staff and students should prioritise the logging of reports, including incidents which have resulted in an injury, to support ACU to reduce health and safety risks. Timely logging of reports may also assist staff members to quickly access worker’s compensation management and support to achieve a quicker recovery from injury. Staff should ensure they are prompt in contacting HR Advisory ( or ext. 4222) if they wish to make a worker’s compensation claim.

Nominated supervisors are sent notifications of reports

Nominated supervisors and other staff are notified by email when reports of incidents, injuries or hazards are submitted. These reports are then reviewed by these staff members and action plans are developed within riskware. This supports the University to either eliminate the hazard or reduce the risk of another incident occurring.

Coordinating a response

Nominated supervisors may contact their local Facilities Management team for support, if a report identifies a property or facilities risk such as a broken step or railing. Nominated supervisors should also consult with their team about proposed control measures (treatments) to ensure that everyone is committed to applying the changes.

Annual reviews of Risk Registers

Some of the risks which are identified by riskware reports may be placed on organisational unit, campus and University risk registers during the annual review of risk registers.
