This form should be completed by ACU students applying for re-admission to the University following a period of termination/exclusion/suspension.

Re-enrolment is subject to approval by the relevant Head of School.

Note: Not all courses offer a Semester 2 intake. Check Courses to ensure your course is being offered prior to submitting this form (mid-year availability normally published in late April).
