Candidature Changes Forms
- Convert EdD to PhD
- Upgrade Master's Degree to PhD
- Variations to Candidature (including leave of absence, course-load changes, extensions)
- Change of Campus, School, Supervisor or Topic
- Resumption of Candidature
- Notice of Intention to Submit a Thesis
- Thesis Allowance (claim form for APA and ACUPA Scholarship holders only)
